2. Puppy must be no older than 16 weeks old at the start date of the class. (Exceptions might be made for small or miniature breeds only. Please call before registering if you would like to request an exception be made for your dog).
3. Puppies are usually too young to be fully vaccinated for this class. They do however have to be up to date on their vaccines and we need our completed form to be filled out and signed by puppy's Veterinarian.
Please note: veterinarian letter will be retained for Dog Gone Good 's records. A negative fecal test within two weeks from the start date of class is also required.
PLEASE BRING COMPLETED AGREEMENT FORM AND LETTER FROM YOUR VETERINARIAN TO THE FIRST CLASS.

Class Requirements
1. Negative fecal test within the past six months
2. Up to date on all vaccines by the first day of class including rabies, DHLPPC, and Bordatella. Please note:
a. puppies must have received final set of puppy shots, which is usually around 16 weeks of age.
b. most vets do not automatically give the Bordatella vaccine, therefore you usually need to request it.
c. The Bordatella vaccine is required every 6 months, not yearly
3. Completed agreement form signed by all adults in the household.

4. Dogs must also be dog and people friendly to attend any group class.
PLEASE BRING COMPLETED AGREEMENT FORM AND A COPY OF PROOF OF VACCINES AND NEGATIVE FECAL TEST TO THE FIRST CLASS. ADMITTANCE TO CLASS WILL NOT BE ALLOWED WITHOUT SIGNED AGREEMENT AND ADHERENCE TO CONTENTS, AS WELL AS CLASS PAYMENT AND PROOF OF REQUIRED VACCINATIONS/FECAL TEST.
Please note: 1. Vaccination records will be retained for Dog Gone Good's records. Please do not bring puppy passport. We need to have a copy of your receipt from your vet visit. 2. Please be sure to also click on your class of choice above (FP1, FP2, etc.) for important information you will need to know before your first class.
